If W-2 employed

• Driver's license or picture ID
• Last 30 days of paystubs
• Last 2 years of W-2's
• Last 2 months of bank statements for all accounts (all pages)  
• 401k statements, Stocks statements, investment accounts, etc.
• Social Security and/or Pension award letter (if applicable)

If self-employed will also need 

• Last 2 years of personal tax returns (all pages and schedules)
• Last 2 years of 1099's (if applicable)
• Last 2 years of corporate tax returns (all pages and schedules) 
• Last 2 months of business bank statements for all accounts (all pages)
